Finding an affordable place to live can be a challenge, especially for families. At Model Cities we offer stable housing with services that help residents build the life they imagine. Services are delivered with care and respect and are focused on young adults with children.

Rental process
How Do I Get a Referral?
To live in our supportive housing, families must first have a referral from Ramsey County.
Supportive Housing referrals for our Families First and Sankofa programs must come from a Ramsey County CAHS case manager. To contact CAHS, please call 651-291-0211 or call United Way’s Helpline 2-1-1.
Families First Apartments
Strengthening neighborhoods and families
Model Cities has been offering supportive housing to families experiencing homelessness for 28 years. Our first multi-family development, Families First, was completed in 1992, at 515 Dale Street. Since then, we have acquired and constructed 32 additional units in five multi-family apartment buildings.
HUD requirement
Families First provides safe, affordable, permanent supportive housing for single-parent families who meet the HUD definition of homelessness. This includes at least one family member with a documented disability.

Sankofa Apartments
Giving young parents more stability
Sankofa provides permanent supportive housing for youth and young adults ages 17-25 who are either pregnant or parenting a child under the age of three years old.
Long term homelessness requirement
Households served through Sankofa must meet the State of Minnesota’s definition of “Long Term Homelessness,” which includes households who lack a permanent place to live continuously for a year or more, or at least four times in the past three years.
