Crisis Intervention

Crisis Intervention targets families who live in Ramsey County and with whom there is a child at risk of out-of-home placement or in the process of family reunification. Specifically, the program works to control or eliminate the risk factors that families face, stabilizing the family and maintaining or reestablishing family cohesion. These services are tailored to meet the immediate needs of the family or adult, and are intended to reduce the number of out-of-home placements, increase family stability, and preserve the family unit.

 

Model Cities accepts referrals from Ramsey County Community Human Services for the Family Community Partnership – Parent Support Outreach Program (FCP/PSOP).  FCP/PSOP is a voluntary program that assists low-income families with identifying community resources to meet their basic needs. Through the FCP program, parents are engaged in selecting the resources and services they need to support healthy family development and increase their capacity to adequately care for their children.

 

Services provided through FCP/PSOP includes the following:

  • Short-term case management services;
  • Assistance with basic needs;
  • Connection/referrals to community-based resources;
  • Emergency fund assistance.

 

Eligibility requirements for the FCP/PSOP program are as follows:

  • The family must reside in Ramsey County;
  • The family is not currently receiving services from Ramsey County Child Protection, Children’s Mental Health, Adult Protection, or Children’s or Adult’s Developmental Disabilities Units;
  • The family has at least one child age 18 or younger.
