Children’s Mental Health Case Management

In partnership with Ramsey County Community Human Services, Model Cities provides Children’s Mental Health (CMH) targeted case management services. These services are designed to support children with severe and long-term mental health needs to receive health, social, school, educational, employment, and other necessary services. Model Cities’ CMH case managers work closely with families to develop services plans, identify community supports and advocate for the needs of the child. To be eligible for CMH services, children and families must meet the following criteria:

 

  • Child or adolescent has been diagnosed with severe emotional disturbance (SED);
  • Child or adolescent are under the age of 18;
  • Family is a resident of Ramsey County.
