Job Description:

The Housing Case Manager provides professional-level service coordination and case management for families who have experienced homelessness and are living in permanent supportive housing. Housing Case Manager is responsible for maintaining a caseload of up to 20 households at any given time. Case management is specifically focused on assisting families in achieving increased housing stability and self-sufficiency by assessing for services and implementing the services based on the families’ needs and goal plans.

Our Supportive Housing Team has years of experience supporting families to achieve their personal goals and maintain housing stability. Our work environment requires critical thinking combined with the ability to hold compassion and accountability for self and others.
As a Housing Case Manager, you will assist families who have experienced homelessness from initial entry into our housing program, throughout tenancy and ultimately upon their transition to new stable housing. Our supportive housing programs serve families with the greatest barriers, to assist them in reaching their greatest successes.

Housing Case Manager should demonstrate cultural understanding, respect, resilience, adaptability, conflict resolution, and resourcefulness. In this role, interpersonal skills with the ability to build trusting relationships through collaboration will be vital. The Housing Case Manager will be under the direct supervision of the Program Manager.

A Housing Case Manager provides intensive case management services which includes but is not limited to needs assessment, case planning and management and documentation of services provided.

 

 

JOB DUTIES:

This position requires the ability to multi-task in the following areas:

  • Providing intensive case management and outreach to homeless families;
  • Conducting assessments for program participants and developing and monitoring family support plans to address problems/concerns;
  • Performing weekly home visits to ensure families are reaching their desired goals; Providing advocacy and coordination of supportive services (i.e. mental health, chemical health, parenting support, financial capability and employment training).
  • Developing and maintaining client data and records for all program participants.

 

Minimum Qualification:

  • Bachelor’s degree in social work or related human services field.
  • At least 2 years of direct services experience working with high risk populations and/or special needs populations that are culturally diverse.
  • In lieu of a Bachelor’s degree, must have at least 6 years of housing case management or closely related work experience. Previous experience working in an urban community and nonprofit environment is a plus.
  • Qualified applicants must have a valid driver’s license, automobile and valid automobile insurance. Must have a good driving record and be able to transport individuals as needed. Background Check required for this position.

Compensation: Low 40s –Mid 40s, depending on qualifications and experience.
