Program Summary
Model Cities’ Sankofa program provides supportive housing with services to homeless youth and young adults between the ages of 17-25 who are pregnant or parenting an infant child. The ultimate goal of Sankofa is to reduce repeated episodes of homelessness, help young parents’ live healthy, productive, and self sufficient lives.
How We Support Families
Sankofa’s range of supportive services assist participants in achieving self-sufficiency; strengthen families by creating, nurturing and financially stable environments for children; link families to quality health care; and promote education as a means to individual success and gainful employment. Core services include the following:
Supportive Housing includes fully furnished sober housing, and comprehensive support services for the young families.
Independent Living Skills Development includes skill building activities for daily responsibilities such as budgeting, time management, and housekeeping.
Early and Continuous Primary Care includes medical and dental services, health education, and public health nursing services provided throughout the family’s participation in the program.
Career Development services include Adult Basic Education and GED courses, communication skill-building, soft skills training, resume writing, completion of job applications, and job search assistance.
Family Support Services includes crisis intervention, parenting education, family therapy, coordinated family activities and access to community resources.
Client Assistance provides financial relief and emergency assistance to families for food, health care, and utilities.
