Children’s Mental Health Case Management

Children’s Mental Health Case Management Services help children with severe and long-term mental health needs to receive health, social, school, educational, employment, and other necessary services. Case Management connects and coordinates the family’s mental health or other services, organizes a wraparound team, and assesses progress over time. Model Cities works in partnership with Ramsey County Community Human Services to provide CMH services. To be eligible for services, children and families must meet the following criteria:

  • Child or adolescent has been diagnosed with severe emotional disturbance (SED)
  • Child or adolescent are under the age of 18;
  • Family is a resident of Ramsey County

Model Cities’ CMH case managers are experienced workers with mental health needs. Staff meets with families to develop services plans, identify community supports and advocate for the needs of the child. CMH requires cross collaboration and planning with schools, mental health providers, probation, respite and social service providers.
